The Community Planned Care Team visit customers in their own homes daily and provide person centred care and support to people who may have a physical disability, mental health condition or learning disability. Support can include the provision of personal care, medication support, financial support and social support in the community.
The service operates a set rota featuring the following shifts:
7am – 3.30pm, 1.30pm – 10pm and alternate weekend working (no overnight shifts are required)
You will be required to work independently in the Community and have the use of your own vehicle with business use insurance.
The job role includes working shift patterns including weekends and bank holidays.
