An exciting opportunity has arisen to join the Recovery and Independence Team as a supervisory recovery assistant. The supervisory role duties include supervision of staff, undertaking initial visits, assessment, support plans and risk assessments of service users in their own homes, providing personal care support to service users, management and administration of medication, taking physiological measurements, dealing with incidents etc.

The Team provides a short term (for up to six weeks) reablement service to people in their own homes which enables people to regain as much independence as possible in skills for daily living ( personal care, meal prep, medication, mobility, use of equipment, accessing the community etc).

The team also provide a rapid response service to vulnerable adults in crisis.
This is a short term Care at Home service (up to seven days) to support hospital discharge and prevent admission to hospital or a care home.
The working pattern is covering variable shifts working 3 days week 1 and 4 days week 2 and this includes a 30 minute unpaid break totalling 50 hours over a 2 week period.

The successful candidate will: hold a minimum of a level 3 Diploma in Health and Social Care or equivalent; hold the HSC level 3 Medication Unit or have a commitment to achieve the unit; numeracy and literacy level two or equivalent; clinical skills level 3 or a commitment to achieve the qualification, have experience of working in a care/health setting; have an understanding of the ethos of reablement, drive and have the use of a vehicle.
