Gateshead Council is a visionary and ambitious Local Authority - ambitious for Gateshead the place, and for the people who live and work here. We have made significant investment into our Adult Social Care Service.

As a result, we have created a number of new and exciting roles to manage the Council’s approach to its residents ‘Living Thriving Lives’, as part of the Council’s Strategic Approach ‘Thrive’.

We are seeking to appoint two full time Social Workers to work within our Achieving Change Together (ACT) Team. This team works with people who have long term and complex care needs, often having received support for many years. The focus of the team is one of long term enablement, supporting people to move from restrictive care arrangements to more independent lives – helping them to ‘Thrive’.

You must have knowledge of relevant legislation including The Care Act 2014, Safeguarding Adults Legislation Policy, The Mental Health Act (amended 2007) and its interface with The Mental Capacity Act 2005, Deprivation of Liberty Safeguards and Continuing Health Care and section 117. You will have the ability to interpret this legislation and apply it to practice while adhering to the Local Authority's statutory responsibilities.

You will have a Social Work qualification (MA or BA in Social Work, Dip SW, CQSW or CSS or equivalent) and be registered with Social Work England. You will have experience of working with people (and their carers) who have needs related to Mental Health, Physical Health needs and Learning Disabilities.

You will be skilled in complex and person centred assessment work, applying relevant criteria and where necessary identifying appropriate services to meet eligible needs, working closely with commissioners, partners and care providers in arranging care and support, and have an understanding of the importance of the reviewing and monitoring process in supporting people to reach goals.

You must have the ability to analyse information, manage risk and levels of need in line with eligibility criteria. As well as experience of managing risk and being involved in the safeguarding process.

You will have effective communication skills and be committed to equal opportunities, anti-oppressive practice and the Human Rights Act. You will have excellent IT skills and the ability to prioritise your workload and diary and manage competing demands. Maintenance of the Local Authority's social care database including the accurate recording of case observations is essential.

It is essential that you are able to work as part of a multi-disciplinary team and use your own initiative and have the ability to identify when support, advice and guidance is required. It is also essential that you have a current driving licence and access to a car or means to mobility support.
