Newcastle City Council (NCC) have been successful in applying for funding to the Public Health England Rough Sleeping Grant: Testing Community Based Models of Access to Health Services.

The funding will enable a Dual Diagnosis Team (DDT) to be established that will work to provide specialist support and interventions to people rough sleeping or at risk of rough sleeping who experience mental health issues alongside substance misuse. The service and team will be developed in collaboration with our local mental health trust Cumbria, Northumberland, Tyne and Wear NHS Foundation Trust (CNTW), and commissioned services that support people rough sleeping or at risk of rough sleeping.

The DDT will consist of two mental health social workers (employed by NCC) and two Dual Diagnosis Link Workers (employed by CNTW). This team will seek to increase access to primary care and specialist services for people who have either disengaged or have difficulty in accessing health care due to their multiple and complex needs by working closely with partner providers to support prevention of evictions as well as with the co-hort of rough sleepers currently identified.

We are looking for social workers who have an interest in working with people who are difficult to engage due to chaotic lifestyles complicated by substance mis-use and mental distress. Experience of working with homelessness and the ability to work creatively within a team will be an asset.

The post is fixed term for a period of 12 months with a start target date of February 2020.
