This role is covered by the Health and Social Care Act 2008 (Regulated Activities) (Amendment) (Coronavirus) Regulations 2021, which is due to come into force on 11th November 2021.

In addition to standard pre-employment checks, an employment offer will be conditional on evidence of a completed course of an authorised vaccine or clinical medical exemption.

The Learning Disability and Transitions Service are excited to have secured funding for an additional 30 hour permanent Social Work post.

Workers in the Learning Disability and Transitions service are enthusiastic, friendly and committed to improving outcomes for people with learning disabilities aged 17+ in Middlesbrough.

This is an exciting opportunity to be part of a specialist team providing comprehensive, multi-agency support to the adults with Learning Disabilities.

The team covers the whole of the Middlesbrough area.

As a member of the team you will have a varied caseload and work with colleagues who are knowledgeable, supportive and passionate about their work.

You will:-

Have a professional Social Work qualification and be registered with Social Work England.

Have previous experience of working with people with Learning Disabilities and their families.

Have knowledge of the available resources in Middlesbrough and our surrounding area.

Need excellent assessment, planning and care management skills.

Need to be able to manage a caseload and prioritise your work.

Have a knowledge of the MCA and other relevant legislation, its implications for this service user group and be able to complete work relating to this.

Be focused, creative and enjoy meeting and working with service users and their families.

Be able to work alongside colleagues in a positive professional environment to potentially include home and office working and will be involved in the development of services for this vulnerable group.

Have the skills to liaise with professionals in Education, Health and the Voluntary Sector to ensure a holistic approach is followed to meet the needs of disabled people.
