Further information

An opportunity has arisen for a suitably qualified and experienced Social Worker to join Darlington Borough Council’s Adult Social Care Department in the First Point of Contact Team.

The successful applicant will work as part of a team of Social Workers and Community Assessment Officers, responding to initial contacts into Adult Social Care, such as requests for assessments of need, enquiries regarding safeguarding issues, advice and information about local services, and responding to urgent or crisis situations.

Plans for piloting rotational work with the Short Term and Review Team, will enable the provision of short term Social Work with adults aged 18+ who have a range of complex needs in the Darlington area. This will involve working proactively with service users, their families, carers and other relevant professionals to maximise independence through a comprehensive strengths-based approach to assessment and support planning.

The successful applicant will also be responsible for completing a range of assessments including Care and Support needs, Mental Capacity Act 2005, Safeguarding, and Support Planning and promoting the wellbeing of vulnerable adults in line with the Care Act 2014.

We are looking for someone who is committed to strength based and person centred practice, focusing on enabling people as much as possible to build on their own networks and strengths as a means of resolving situations and meeting their needs.

The successful applicant will also be able to, through gathering information from initial contacts, make appropriate and sound decisions as to what action needs to be taken, such as referring people for assessments under the Care Act 2014, as to whether safeguarding issues are indicated, or signposting people to partner agencies and services.

The successful applicant will:
• Have sound assessment skills and be able to respond to initial contacts appropriately
• Be able to work in a busy environment and to tight time scales
• Have excellent written and verbal communication skills
• Be committed to multi-disciplinary working

Our team offers regular supervision and appraisal, excellent training opportunities and strong team identity.

This is an excellent opportunity, not only for individual development, but to be part of a very proactive and forward thinking team who will play a key part in terms of providing a first class response to those who make contact with Adult Social Care.

Applications are invited from suitably qualified individuals.
