An opportunity has arisen within the Transition Team in Care Well-being and Learning (CWL) for a permanent Social Worker 22hrs 12mins per week. We are seeking to appoint an experienced Social Worker with relevant post qualifying experience.

You will have a CSS, CQSW, DipSW, Social Work degree/ masters or equivalent, and be registered with the HCPC. You should also have a post qualifying award in Social Work.

You will have experience of working within Adult Social Care teams in particular with clients whose primary need is a learning disability and/or Autism or physical disability.

It is essential that you have knowledge of relevant legislation including: Care Act 2014, Safeguarding Adults Legislation Policy and Guidance, Mental Capacity Act 2005 and Deprivation of Liberty Safeguards. Children's legislation such as The Children and Families Act 2014 and the Personalisation Agenda The ability to interpret this legislation and apply it to practice whilst adhering to the Local Authority's statutory responsibilities within everyday practice.

You must have experience and knowledge of the SEND code of Practice, of (EHCP) Education, Health and Care needs assessments and plans and of the Court of Protection process.

You will have direct experience of undertaking relevant assessments including risk assessments associated with the role, including reviewing and applying relevant criteria when assessing needs and completing Mental Capacity assessments.

You will demonstrate the ability to analyse information, assess risk and levels of need, in line with safeguarding policy, practice and eligibility. Alongside providing support and mentoring to colleagues.

You must have the ability to use your own initiative, manage and organise your own workload, including the ability to prioritise tasks, and manage competing demands, as well as working as a member of a busy team.

You will be experienced at providing Education & Training for both children and adults.

You must be able to demonstrate your ability of solving problems and making appropriate decisions.

You will have a flexible approach and willingness to move to another team as and when required.

The right candidate will have excellent IT skills and be able to use the local authorities social care database, including case recordings and relevant electronic documentation.

You must have good interpersonal and communications skills and the ability to demonstrate a commitment to anti-oppressive practice.

You must have a commitment to continuous professional development and a willingness to undertake relevant training as required by the local authority.

You must hold a current UK driving licence and access to a car or means of mobility support.
