We have an exciting opportunity for a full time permanent Social Work Policy and Practice Manager. You will be an experienced registered social worker and you will be responsible for working with our social work teams across a range of service specialities to facilitate and support with complex care packages, providing scrutiny and specialist input to high cost packages. The post holder will lead on identifying and sourcing suitable providers, scrutinising and validating costs, ensuring that where appropriate, Health funding has been secured, Assistive Technology has been considered and that Mental Capacity issues have been adequately addressed, prior to funding for individual support packages being agreed and services commissioned.

The post holder will ensure that systems are in place to enable Adult Social Care in conjunction with other relevant partners fulfils it statutory duties and responsibilities in terms of compliance with relevant social care legislation and guidance in relation to the Mental Health Act.

The post holder will work with colleagues in health to ensure that appropriate policies are agreed and funding arrangements are in place in respect of NHS Continuing Health Care and Section 117 Aftercare, and that appropriate governance arrangements are in place regarding the Local Authority’s duties relating to CHC Section 117. The post holder will also be responsible for monitoring performance in relation to CHC and 117, and for preparing reports for the Director of Adult Social Services (DASS).

The post holder will be responsible for leading with the interpretation and dissemination of key social care and health guidance and policies from the Department of Health and other national agencies in respect of all client groups, which will impact and/or will shape future care management practice or commissioning decisions. The post holder will work closely with operational managers to ensure that National legislation and policy is well embedded within the assessment teams.

The post holder will act as a reference point for practitioners, offering a wide range of specialist advice and guidance.
