If the answer is yes to any of these, North Tyneside Council wants to hear from you. We’re recruiting for a permanent, (37 hours) Community wellbeing officer post for Care Point.

Care Point is an integrated health and social care team based at North Tyneside General Hospital you would be part of the team of social workers and community wellbeing officers facilitating safe discharge from hospital wards in North Tyneside General Hospital and out of area hospitals for North Tyneside Patients. You will be working with health colleagues and working in multi-disciplinary teams supporting patients discharge from hospital, completing assessments and support plans.

Within care point we work closely with our health colleagues and reablement service also preventing admission to hospital

We’re using a strength-based approach to help people transform their lives, which will focus on the community they are part of and that empowers them to take as much control of their lives as possible.

We have a well-established programme for training support, and you’ll be given free access to range of online and face to face resources to help you evidence and develop your skills and practice.
