The opportunity has arisen for a suitably experienced Community Assessment Officer to join Darlington Borough Council’s Adult Social Care Department in the Adult Contact Team. The team is based at Central House in Darlington and provides a short term social work service for adults aged 18+ with a range of complex needs in the Darlington area.

The team, comprising of both Social Workers, Community Assessment Officers and Occupational Therapists, works proactively with service users, their families, carers and other relevant professionals to maximise independence through a comprehensive strengths-based approach to assessment and support planning. The dynamic, fast paced nature of the team is supported by a strong commitment to the principles underpinning The Care Act 2014 and other relevant legislation.

The successful applicant will be required to contribute to the operational delivery of an effective Adult Social Care Service and be responsible for a range of less complex assessments including care and support needs, Safeguarding, Support Planning and promoting the wellbeing of vulnerable adults in line with the Care Act 2014. Promoting independence and wellbeing ensuring desired outcomes are achieved through person-centred planning/strengths-based assessment.

The successful applicant will have approximately 3 years’ experience of working in a Social Care/Health field and will demonstrate:

• Sound assessment and care management skills
• Ability to work in a busy environment and to tight time scales
• Excellent written and verbal communication skills
• Commitment to multi-disciplinary working

Knowledge and experience of assessment processes and legislation would be desirable.
